1. Welcome to GAFSC's Community Needs Assessment

Thank you for your interest in the Georgia Avenue Family Support Collaborative (GAFSC), a DC community-based organization in Ward 4.  GAFSC is dedicated to supporting children, youth, and families to become healthy, empowered, and connected in a multicultural community. Its services and programs focus on family support, rehousing and housing stabilization, child abuse and neglect prevention, workforce development, truancy prevention, and youth support and development.

GAFSC is conducting a Community Needs Assessment of its target populations and neighborhoods in Ward 4.  You must be an adult, 18 years or older, and reside in Ward 4 to complete this survey.  By completing this 20 – 25 minute survey, you help GAFSC understand families' needs, resources, and the impact of COVID-19 on them and their community. 

We will use the survey findings to plan our priorities, services, and programming.  We will also share this information with funders, policymakers, other community-based organizations, and DC government agencies whose programs and services affect your family's wellbeing. 

Confidential Statement:  All responses to this survey are anonymous; responses cannot be identified or connected to any specific person. Therefore, there are no risks for participating in this study and no penalties for not participating.
