Hospital Discharge Experience Survey

Have you, or someone you care for, been sent home from hospital in the last 12 months?

We want to hear about your experience.

Whether you were sent home from a main hospital such as Worcestershire Royal Hospital, Alexandra Hospital or Kidderminster Hospital, or from a community hospital such as Evesham, Pershore, Tenbury, Malvern or Bromsgrove Community Hospital, your feedback matters.

We're also keen to hear from Worcestershire residents who were sent home from hospitals outside the county.

Being sent home from hospital is an important part of a patient's journey. When it goes well, people can recover with confidence. When it doesn't, it can leave people and their families feeling anxious, unsupported or unsure about what happens next.

By sharing your experience, you'll help us understand what's working well and where improvements may be needed for people across Worcestershire.

The survey should take around 5 minutes to complete, and all responses will be anonymous.
