Client Satisfaction Survey

This feedback form has been developed to enable Psychology & Support Services department (P&SS) to collect information and assess the effectiveness and level of satisfaction with the Retired and Former Employee (RAFE) Peer Support program. The information you provide will be used to ensure that procedures, protocols and Peer Support training are meeting the needs of AV employees and their family members.

Please note the following:
  • This survey is anonymous and cannot be traced back to you personally unless you choose to complete the sections concerning the date of the incident, your name or the name of the Volunteer RAFE Peer that made contact with you.
  • Participation in this survey is voluntary however, supplying this information assists the PSS Peer Coordinators to address any negative feedback with the Volunteer RAFE Peer concerned, thereby improving our level of service.
  • This survey will take around 5 to 10 minutes to complete.
  • Please complete a feedback form for each separate contact that you have with a Volunteer RAFE Peer. All data will be secured in accordance with AV guidelines.
  • All electronic data will be deleted after the required six year retention period.
  • You are free to withdraw your consent at any time during the survey in which event your participation in this survey will immediately cease and any information obtained from you will not be used.
