Hello, Troop Leader! The purpose of this questionnaire is to help us to prepare for the 48th Annual Girl Scout Day RESCHEDULED for September 22, 2018. Your response will help us to determine expected attendance overall, the number of scouts to expect at each program, and the age ranges of participants.

Please note that troop leaders are not required to complete this questionnaire in order to participate in Girl Scout Day, but we strongly recommend it - so that we are able to have adequate space and supplies for each activity.
