Participant Information Statement & Consent

This study is being undertaken by The University of Wollongong and The Grains and Legumes Nutrition Council (GLNC), a health promotion charity. Click here for more information about GLNC.

This is a formal invitation to participate in research about breakfast.

The aim of the study is to assess how breakfast is changing as a meal within the Australian diet and review consumer preferences.

For your participation, we are offering the chance to win an inspirational breakfast prize pack worth $50 thanks to the Grains and Legumes Nutrition Council.

Participant Inclusion Criteria
Participants will require the following to be considered for inclusion in the study:
·       You are required to be over 18 years of age.

Description of Study
·       If you agree and consent to participate in the study, you will be asked to complete a 5-10 minute anonymous online questionnaire that will then be interpreted by the investigator remotely.
·       The opportunity for participants to partake in this online survey will be open for approximately 5 weeks depending on response numbers, whereby any person can opt to complete the survey.
·       The survey will ask questions about your breakfast preferences and habits.
·       Participation is voluntary and confidential. Your time and participation is greatly appreciated. At the end of the survey, you will be asked to provide your email if you wish to be entered into the prize draw.

Requirements of Consenting 
Participants will be expected to:
·       Donate 5-10 minutes of their time to complete the online survey to the best of their ability.

Confidentiality and Disclosure of Information 
The survey is via an online tool and responses will be electronically recorded. For the purpose of the prize draw, you may add your email address, but this will be removed from the survey information provided and discarded once the prize winner is drawn. All survey information will be stored on a password protected laptop that only named researchers have access to. At no time will your actual identity be available. In any publication, information will be provided in such a way that no participants can be identified.

Your Consent
You may withdraw from participating by exiting the survey at any point before completing and submitting the online survey, if you wish to do so. Incomplete survey information provided will not be stored or used in the findings of this study. If you decide to participate, all survey responses will be collected and once data is submitted it cannot be withdrawn due to the anonymous nature of the survey.
