ABOUT THE SURVEY
The LGBTQ Parenting Network at Sherbourne Health Centre is undertaking a program evaluation. Consultants Jasmin Earle and Sharon Kelly are undertaking this evaluation on behalf of the Parenting Network.
The intent of this survey is to ensure that the Parenting Network's future priorities and services are informed by evidence of need. We are gathering information from key organizations and people with whom the Parenting Network has a relationship, in order to identify external perspectives on the needs of the community, and the needs of the Parenting Network's partners and clients. As a parent or a prospective parent who has used Parenting Network services, your views are very important.
The results of this survey will be analyzed by Jasmin Earle and Sharon Kelly and will be shared with the Parenting Network in the aggregate. Survey responses will be anonymous. We will not identify the responses received from specific respondents.
The survey should take approximately 15 minutes to complete. Please submit your responses by November 8th. In the event that you must leave the survey prior to completing it, simply exit the survey and the next time you return to the survey you can pick up where you left off.
