Thank you for participating in our survey.
In order to progress with this survey first read the participant information on this page including the consent questions and then complete Question 1 as the form of consent.
My name is Dr Duncan Reid and I am a Researcher of the Auckland University of Technology Sports Performance Research Institute New Zealand (SPRINZ). My research interests are in the area of sports related head injuries and concussion.
The Accident Compensation Corporation (ACC) in New Zealand has recently released a guideline to help with the management of sports-related concussion.
Invitation to participate:
· You are invited to take part in the above mentioned research project. Your participation in this research is voluntary. You are free to withdraw consent and discontinue participation at any time without influencing any present and/or future involvement with the Auckland University of Technology.
· Your consent to participate in this research will be indicated agreeing to complete the attached anonymous survey. Ticking the consent box at the start of the survey indicates that you have freely given your consent to participate, and that there has been no coercion or inducement to participate by the researchers from Auckland University of Technology.

You are currently practicing physiotherapist in New Zealand
What will happen in this research?
· All you will need to do is complete a survey electronically. The survey will take about 10-15 minutes of your time. The survey is anonymous in nature.
What are the benefits?
· Information gained from this research will help us better understanding or your knowledge of concussion and also whether the ACC guidelines have had an impact.
How will my privacy be protected?
· Your privacy is completely protected as at no point will we collect your personal details like your name. The survey in fully anonymous. The data collected will be held in secure storage at SPRINZ (at Auckland University of Technology) the responsibility of the main researcher, in accordance with the New Zealand Privacy Act (1993).
What are the costs of participating in this research?
· Participating in this research project will not cost you apart from your time that you provide to complete the survey.
What opportunity do I have to consider this invitation?
· You have a week to consider the invitation but if you are happy with the information then you can proceed at any stage you like to the electronic survey.
How do I agree to participate in this research?
You can discuss the survey at any time with the researchers to ensure comfortable with the purpose of the research.
