We greatly value the support of our donors.  In order to ensure that your needs are being served and that you are confident that the KGH Foundation is investing your generous gifts wisely, we have put together this brief questionnaire.  Thank you for taking the time to help us serve you better.

All information will be kept confidential and used for the sole purpose of improving how we work with you. Please feel free to omit any questions you don not feel comfortable answering.  
1.Each year we distribute newsletters, an Annual Report and opportunities to support the KGH Foundation.  How would you like us to communicate with you?
