Nomination Guidelines
Nomination information must be complete to provide sufficient substantiated information to assist the Awards Advisory Committee in forming a fair assessment of the candidate.
Each award has different terms of eligibility. Nominees who do not meet the specific terms of award will be disqualified.
Nominators are encouraged to be selective of supporting documentation, and present significant information clearly.
Once supporting documentation has been collected, the nomination form will take approximately 10-20 minutes to complete.
