Park District of Forest Park Community Survey

The Park District of Forest Park is conducting a Community Survey to help provide direction for the next five years. Please take 10-15 minutes to answer the survey questions below. Your feedback will allow us to provide activities and make park improvements to serve residents and other park users.

In answering these questions, you may want to get input from other members of your household who also use park facilities. As clarification, this survey covers programs and facilities located at the Park on Harrison Street only.

Survey responses are confidential. All responses will go to an independent research firm. Park District Board Commissioners and Staff will not have access to individual responses. As a thank you for your participation and quick response, we will be awarding 10 family pool passes for 2016 (up to 4 people per household) in a March 2016 drawing. Once you have completed the survey, you will be asked to fill in your name, email, and phone number if you wish to enter this drawing. This information will only be used for the drawing and will not be associated with your survey responses.
