[PET - 083] For Us Baby Books Questionnaire

We would like to invite you to complete this short survey about your recent experience regarding the For Us baby record book that was provided by the maternity service.

This book has been made to help support parent’s grief and mental health when they have experienced baby loss in pregnancy, at any stage. The book is being given to parents currently as part of a service improvement project, hoping to improve resources for grief and mental health in the maternity service.

All information you provide will be kept strictly confidential. Participation in this project is voluntary and if you choose not to take part, it will not affect the care you receive from the NHS in any way. Please do not provide any confidential information (such as your name or address) in your responses.

Your feedback is very valuable to us so we can continue to provide resources such as these for future parents who experience losing their baby.
