Introduction

This survey should be completed by your homelessness system leadership. This can mean the person in charge of the Continuum of Care, Emergency Shelter Grant administrators, executive directors of homeless service organizations, foundations or funders of homelessness assistance, directors of partner agencies such as the Housing Authority, and local government officials. This survey asks for your opinions on your local homelessness system, also called a Continuum of Care. All responses are anonymous.
