Your child’s school is participating in the FAST Institute for Family Engagement. The FAST Institute is designed to improve family, school, and community relationships. We are asking the families of students at this school to participate in a brief survey regarding their involvement with family engagement. The survey is a set of questions about family engagement and your answers will help us to know if the program is doing what it is designed to do.
It is your choice if you would like to participate in the survey or not. You may choose not to participate and choosing not to participate will not affect you or your child in any way.
You will be asked to complete a survey before the program begins and a survey after the program ends. The survey takes approximately 15-25 minutes to complete. Your responses will be confidential and we do not collect identifying information such as your name, email address or IP address. The survey questions will be about family engagement at your child’s school.
We will do our best to keep your information confidential. All data is stored in a password protected electronic format. To help protect your confidentiality, the surveys will not contain information that will personally identify you. The results of this study will be used to help identify the best ways improve family, school, and community relationships and to support children for success.
