2018 Family Outreach Survey

Perry Central Family Outreach strives to help families. Please help us make our program better by answering these three questions.

Provided activities for our family to do together. (Examples- Candy Cane Carnival, Fall Fest, Family Field Trips, Grandparents Day, Back to School Cook-out)

Provided information on school and community events and Perry County resources. (Examples- Monthly newsletter, brochures, flyers, direct contact)

Helped increase our knowledge on ways to improve student success. (Examples- Grade Level Orientations, Career Day, New Family activities, Prevention programs, Parenting programs, Family Literacy Nights, etc.)

Increased our knowledge on general health and encouraged us to work towards a healthier lifestyle. (Examples- Health Fair, Family Exercise and Nutrition Programs, Community Fitness activities, etc.)

In conjunction with programs, provided free meals, family programming, and childcare. Helped our families in a time of need. (Examples- School Supply Bank, Holiday Helpers, Food Drive, Backpack Food Program, etc.)

New to the community, so we have not had an opportunity to participate yet.
