* 4. LOCKER CONTRACT

Jefferson agrees to provide locker service to applicant.

I understand that I will not place any library materials in a locker that I have not properly checked out from the circulation desk. To do so would violate Thomas Jefferson University’s Code of Conduct (Policy 107.02) and provide an unfair advantage over other students. I also understand that I will not place any unlawful items or items which violate University policy, including but not limited to drugs, weapons, stolen property, hazardous materials, other contraband.

The term of locker use begins when the locker is assigned to the student and is effective for a period no greater than four months. Rental periods will vary each academic year. Students will be made aware of the rental period, including the expiration date at the time of application.

Students may have the option to renew their locker for another four month period if they notify the Office of Student Life and Engagement prior to their current expiration date and pay the current locker rental fee. Locker renewals are not guaranteed and will be governed by the most current locker policy in effect at the time renewal is sought.

If the student does not check out of their locker, nor take action to renew their locker term, the Office of Student Life and Engagement will reclaim the locker at the end of the current rental period and any remaining items will be disposed of. Should the student cease to be enrolled at Thomas Jefferson University at any point the locker will immediately revert back to the Office of Student Life and Engagement and any items in the locker will be disposed of. A hold will be placed on a student’s account for disregard of the rental period and failure to clean out locker or renew by the end of the period. There will be a one-week grace period at the end of each rental period before the locker is emptied and account placed on hold.
The provided combination or key locks must remain on the lockers at all times. The Office of Student Life and Engagement is not responsible for items stored in lockers, but does reserve the right to enter the locker with advance notice to the student. Students are NOT permitted to change the locks used on their locker for any reason without prior written approval from the Office of Student Life and Engagement. A fee of $25.00 will be charged for lost key/lock or use of personal lock.

By typing your name in the box below you are adding your electronic signature. Your locker information will be distributed to you within two business days of receipt of this application
