Howard County Family Leadership Program Application (Fall 2026)

Please complete this application to apply for the Family Leadership Program. Once submitted, a member of the program staff will reach out to discuss your application further. Please remember that space is limited so not all applicants will be accepted into this cohort. If you do not meet the eligibility requirements or are not selected for the program, you will be notified by email.

All responses you provide on this application will remain confidential and seen only by program staff. The information provided though helps us ensure that the program addresses your family's specific needs.

Eligibility
You must be a parent or caregiver to a school-aged child with mental health challenges.
You must be a Howard County resident.
You must be willing to commit to attending both days of the event.

26.This is a 2-day program that has the most benefit when parents and caregivers attend all the workshops held on both days. Do you foresee any challenges attending for the entire 2 day program? Note: If you are unable to commit to attending both days (October 23 and October 24th), consider waiting for the next offering. Preference will be given to those who can commit to attending both days.
27.To promote self-care and provide some respite, MCF covers the cost of the your hotel room for the evening of Friday, October 23rd.

28.MCF can provide limited funds (up to $40/day) to help pay for childcare so that you can attend the event. Please note: Children are not permitted to attend the program. This ensures that all participants are able to gain the maximum benefit from the opportunity to learn, grow, and connect.

The funds will be paid to a childcare provider of your choice (family, friend, or other). The provider will be required to complete a W9 in order to receive payment.
