Fairview Range Online Patient Portal Survey

Fairview Range Medical Center is always working to make you and your family's health the best it can be. In this ongoing dedication, we are learning more about your feelings regarding an online patient portal. Please take 3-4 minutes to take this survey and for finishing it, you will be entered into a drawing for one, of four, $50 Amazon® Gift Cards.

This drawing will be held on July 15, 2017 and you will be notified by either phone or email (your preference). The winner will be announced on Facebook shortly after the drawing.

Self-scheduling – schedule a clinic appointment with your provider online

E-visits – can be used for minor medical problems or managing on-going health conditions. Just answer some questions about your health concern, then hit submit. You’ll receive a response from your provider within 1 day.

Video E-visits – a scheduled, virtual visit with your provider that you can attend from your desktop or cell phone.

Fast pass – you will be notified by text if you are on a waiting list and an appointment opens up, which you can choose to accept or decline via your cell phone.

E-check-ins – confirm address and insurance details online so when you arrive you can simply inform the front desk you have arrived and have a seat in the waiting area.

Message your care team – send a secure message to your provider when you have non-urgent questions related to your care.
