Dear Cheshire Homes Society of BC Client,

You are receiving this survey because you are currently receiving services from Cheshire Homes Society of BC (CHSBC). We are using this survey to gain feedback about our programs and services, and to make sure that we are meeting your support needs in a meaningful way. This is part of our ongoing commitment to continuous quality improvement.

Please note that:
a)  Your participation is voluntary. You do not have to participate in this survey if you don’t want to.

b)  We want to assure you that your responses are completely anonymous. Responses to anonymous surveys cannot be traced back to the respondent. No personally identifiable information is captured unless you voluntarily offer personal or contact information in any of the comment fields.
c)  You are not required to answer each question; however we encourage you to do so.
d)  Your answers will not affect your ability to access services at CHSBC
e)  The 2022 Satisfaction Surveys will close on July 1st, 2022.
