Consent Form for participants taking part in Online Survey

·         I have read the Parent Information leaflet and have had time to consider whether to take part in this study. I understand that this is a study of parents’ experience of living with a child with ADHD who may have coexisting difficulties and the health and education services they receive.
·         The study is being undertaken by Andrew Kennedy in the School of Education, University College Dublin, under the supervision of Professor Gráinne Ní Dhomhnaill for the degree of Doctorate in Educational Psychology.
·         I understand that my participation in the study is voluntary.
·         I understand that once I submit my anonymous data online I will no longer be able to withdraw this data as it will not be possible to identify me, unless I provide my contact details for the opportunity to participate in an interview or focus group.
·         Any information collected from me will be stored in a password protected folder on a password protected computer in a secure location in Andrew’s home.
·         I agree that the data collected can be used in a thesis for the degree of Doctorate in Educational Psychology and in publications.
·         I have had the opportunity to consider asking questions about the study, and if so, I have received satisfactory answers to all my questions
