1.What is your age range?
2.Do you have any physical challenges that would prevent you from frequenting businesses during the Covid-19 era?
3.Would having a proven Covid-19 vaccine available be a major factor for you to return to your "pre-Covid-19" consumer habits?
4.Have you physically visited or shopped a local business during the stay-at-home order? If yes, generally which one(s) retail, restaurant, grocery, service business, etc.
5.Did you feel comfortable in that business? Please explain why?
6.Outside of work, do you engage in zoom and other video communication methods to deliver shopping, fitness, entertainment, educational, etc experiences or are you zoomed out?
7.As the State reopens, businesses must adhere to the Governors Mandatory Business Protocol, but beyond what is expected, what steps can a business take to help you build confidence to visit?   (Check all that apply)
8.Are you willing to comply with additional health, hygiene and cleanliness protocols set up by individual businesses in addition to mandatory state standards?
9.If timed reservation/appointments are important to you, how much time between customers is reasonably comfortable to you so the business can clean?
10.Once able, how likely are you to dine out?
11.Will you feel more comfortable shopping and dining if the experience is outside/curbside?
12.Will you still do “take out” even if you can dine out?
