Welcome to the application for the high school After School Program: Faces From the Past, which takes place on April 17, 2017 from 10:00 am to 4:00 pm. The application deadline is midnight on  April 2, 2017. Please note that your application is not complete until this entire form is completed and you reach the confirmation/thank you page.

Students will be notified of acceptance/rejection via email by April 7, 2017.
