Survey - Shaping the future of Family Voice Surrey

This survey is for parent carers of children and young people aged 0–25 with Special Educational Needs and/or Disabilities (SEND) who are members of, or connected with, Family Voice Surrey (FVS).

As Local Government Reorganisation (LGR) progresses across Surrey, we want to understand how families currently access services, where those services are provided, and what matters most to you as we plan for the future. Your feedback will help us understand the experiences of families across Surrey and ensure that the voices of parent carers help shape FVS's future work and engagement.

The survey should take approximately 5 - 10 minutes to complete.

Your responses will be treated confidentially. Individual responses will only be accessed by Family Voice Surrey and will not be shared in a way that identifies you, unless a response raises a concern about someone's safety.

The information you provide will be combined with responses from other parent carers to help us build a picture of how families currently access services across Surrey, identify priorities for the future, and inform future discussions .

CONSENT STATMENT

By providing consent, you are agreeing that personal information about you will be collected anonymously, stored securely and will not identify anyone personally. The information from the survey will be used to understand the current strengths and potential areas of improvement in Surrey. The anonymised, thematic data may be used and shared with Surrey County Council and relevant NHS partners. Once consent has been provided you have the right to withdraw your views and your views will be removed, however where data has been anonymised and amalgamated into thematic responses in certain circumstances it may not be able to be excluded. 11.We want to ensure that parent carers are kept informed and involved in the charities preparation work for LGR. Please let us know how best we can do this?

To oversee the next stage of Family Voice Surrey's development we will be establishing a steering group in September. We are looking for parents who would be interested in being involved in this work. The steering group will help shape engagement with families. They will also help the trustees to shape what the charity will look like post LGR.
