This survey is for parent carers of children and young people aged 0–25 with Special Educational Needs and/or Disabilities (SEND) who are members of, or connected with, Family Voice Surrey (FVS).
As Local Government Reorganisation (LGR) progresses across Surrey, we want to understand how families currently access services, where those services are provided, and what matters most to you as we plan for the future. Your feedback will help us understand the experiences of families across Surrey and ensure that the voices of parent carers help shape FVS's future work and engagement.
The survey should take approximately 5 - 10 minutes to complete.

Your responses will be treated confidentially. Individual responses will only be accessed by Family Voice Surrey and will not be shared in a way that identifies you, unless a response raises a concern about someone's safety.
The information you provide will be combined with responses from other parent carers to help us build a picture of how families currently access services across Surrey, identify priorities for the future, and inform future discussions .
