FVPSA Performance Progress Report Introduction
The Office of Criminal Justice Services (OCJS) is collecting valuable information regarding the performance of your programs. It is important to OCJS that the information is as accurate as possible so that we can demonstrate the impact that the Family Violence Prevention and Services Act (FVPSA) funding is having for the State of Ohio.
Please note that we are using Survey Monkey. Once you officially start the survey in Survey Monkey, you will NOT be able to save a copy of the report and you will NOT receive a confirmation after the report is submitted. Therefore, OCJS also requests that you print each page before clicking "next" and "submit" so that you have a copy of the report for your records.
