2023 Distinguished Alumni Award Nomination Form

The College of Pharmacy (COP) Alumni Association is soliciting your recommendations for the Distinguished Alumni Award, the highest honor presented by the COP Alumni Association Board of Directors.

Nominees should meet the following criteria:
1. Have distinguished themselves by achieving recognition for academic, scientific or leadership contributions in science, business, public service or otherwise;
2. Have demonstrated exceptional service to their community; and
3. Have shown exemplary support and service to the College of Pharmacy.
