This questionnaire is focused on understanding the key barriers and facilitators of adopting new technologies in sexual health services.

It will take 10-15 minutes to complete this survey.

Refusal to participate in the survey, or failure to answer any of the questions, will not affect you negatively in any way. All information collected during the course of this research will be kept confidential. If you agree to participate you will be assigned a unique Study ID, which will serve as a way for us to anonymously organise your responses. Your email address will not be linked to your survey responses.

All data will be stored on the password-protected Study Database and held by the Study team at St George's University of London (SGUL) on SGUL password-protected computers in an access-restricted file on the secure SGUL network.
