This form must be completed by all nominated high schools before the June 1st deadline. A nominated high school will only need to submit this form ONCE. Include information for ALL of your craft training programs.
Completion of this form is 1/2 of the requirement to participate in the awards program. Nominees will be sent a link to schedule a time to meet with representatives from GBRIA's CWD Committee. This meeting will allow nominees to expand on answers provided in this form and to highlight other elements of their programs not captured in this questionnaire. Further guidelines will accompany the scheduling request.
