Mental Health Matters Award Nomination Form

Every day, mental health and substance use treatment organizations, professionals, and individuals provide essential services to people in need and advocate for themselves. Macomb County Community Mental Health (MCCMH) is honoring their selfless efforts with its Fourth Annual Mental Health Matters Awards. Nominations will be accepted from January 13, 2026, to April 21, 2026, across five award categories.

Eligible nominees must have been involved with MCCMH in the past year. Awardees and their guests will be invited to the dinner event on Thursday, May 21, 2026, to receive their award and be celebrated by MCCMH and local community partners. Winners will be notified no later than May 1, 2026.

To nominate a local champion, select one of the categories below and provide a detailed narrative explaining your nomination. Nominations without details will not be considered.

Self-Advocate Champion – An individual who is currently receiving or has received services from within the MCCMH system and advocates for themselves or others. Nominees must meet the following criteria:
· Impact: The individual must have a history of advocating and interacting with multiple individuals and/or organizations to reach their goals.
· Bravery: The individual illustrates their commitment to advocacy despite challenges that may arise.

Provider Champion – A mental health or substance use provider within the MCCMH system who goes above and beyond in delivering services. Nominees must meet the following criteria:
· Impact: The provider must demonstrate a history of successful service delivery and recent measurable accomplishments that show their positive impact on persons served by MCCMH system of care.
· Innovation: The provider must demonstrate innovation through creative efforts to reach out and provide services in the community.

Caregiver Champion – A caregiver is someone who provides care to a MCCMH person served. Nominees must meet the following criteria:
· Impact: The individual must have a history of exceptional caregiving.
· Person Centered: The individual demonstrates a commitment to providing person-centered care through their tireless efforts to help others achieve recovery goals, participate in the community, and attain self-sufficiency and independence.

Community Partner Champion – An organization that partners with MCCMH to support and advocate for people served. Nominees must meet the following criteria:
· Impact: The organization must have a history of partnership that helps others achieve recovery goals, participate in the community, and attain self-sufficiency and independence.
· Innovation: The organization must demonstrate innovation through creative efforts to reach out and provide services in the community.

Advocate Champion – An organization or individual who advocates for individuals living with mental illness, developmental / intellectual disabilities, or substance use diagnosis. Nominees must meet the following criteria:
· Impact: The individual has demonstrated exemplary efforts and results in supporting others to meet their behavioral health needs.
· Bravery: The individual demonstrates their commitment to advocating for people served, despite challenges and public perceptions.
