Long-Term Care Resources Survey

You are being asked to take 10 minutes of your time to complete a survey to share your thoughts about your experience of supporting a loved one as they moved into a long-term care home. This project is being led by Sam Peck, Liane Pelissier, Cathleen Edwards (Family Councils Ontario Team) and supported by Carol Lemen and other Social Work students completing their placement at Family Councils Ontario. Only members of the project team will have access to the survey data. If you complete the survey, your digital data will be stored securely on the Family Council Ontario OneDrive for a period of 7 years.
