COVID-19 Vaccine Survey for Ohio Schools

Ohio is facing an unprecedented challenge in its fight to slow the spread of COVID-19 transmission. Every region, community and institution in the state has felt the significant impact of this pandemic – including Ohio’s schools. Over the past several months, Ohio’s students and educators alike have demonstrated a tremendous response to the need for wearing masks, washing hands and practicing social distancing. Now, with the development and approval of COVID-19 vaccines, Ohio has another powerful weapon in this fight.

Currently, Ohio is preparing to distribute COVID-19 vaccines statewide in a phased approach. During Phase 1B, vaccine supply is limited, and vaccine administration will be focused on reaching critical groups, which includes adults who work in schools. In order to ensure that vaccine distribution is as efficient and thorough as possible, please complete the following survey by COB on January 7. The information in the survey will allow us to coordinate with you and local partners to ensure that your staff are prioritized in vaccine distribution.
