The following questionnaire has four parts. Each part is critical in the future of First Presbyterian Church Johnson City.

Please answer all questions that you are comfortable completing and make sure to submit your responses before exiting.

Questionnaires must be completed in one sitting so please make sure to have 10 - 15 minutes to complete all four parts.
