Thank you for taking the time to answer a short questionnaire that will assist us in determining the best expansion option we should take.

FOOTPRINTS 4 Autism provides respite for families with children 13 years of age and under who have been diagnosed with an Autism Spectrum Disorder. Respite is provided in a happy and safe environment. Our program is solely geared towards recreation. Many children affected by Autism spend countless hours a week in therapy and this does not provide a lot of time for social play and to just "be". Run as a group day-program, sessions are held on Friday evenings and Saturdays at Dunbarton-Fairport United Church in Pickering and on Saturday mornings at St. Mark's United Church in Whitby. Children see FOOTPRINTS as their own 'social club' - accepted and supported for who they are in a caring and safe environment.

FOOTPRINTS 4 Autism is looking to expand their services and would like to ask families and caregivers a few questions to help us determine what our expansion might look like.
