2022 Conclave: President Attendance Waiver Survey

Basic Information

This waiver form is for those serving as Chapter President in the spring 2022 term and cannot attend the Officer Academy in St. Louis from January 14 - January 16, 2022.

You also do not need to fill out this form if you are not a Chapter President. If you are another officer and cannot attend, but would like to send a replacement, just have the replacement person register. However, replacements for any officers will not count towards receiving the 5% spring census credit.

Please note: by completing this form, your chapter waives its right to a 5% spring census credit as your participation will not be in compliance with the offer.
