What: Maltby Centre is always looking to improve the services we provide to our community. The purpose of this survey is to learn from your experiences with our services so we can make improvements.

Time: It should take about 10 to 15 minutes to complete the survey. 

Privacy and Confidentiality: Your feedback will be kept anonymous and confidential, unless you would like to have a staff member follow-up with you regarding any concerns. Survey results will be reported to larger audiences as a summary so that no individual can be identified. If you choose to provide an email address to be entered into the gift card draw, your email will not be associated with your responses. 

Risks: There are no risks to you if you participate in the survey. You do not have to fill out the survey if you don't want to. Your responses will not affect the services you or your family receive. If questions make you feel uncomfortable, feel free to skip to the next question. You can stop at any time. 

Eligibility: To participate, you must have received services from Maltby Centre in the last 12 months. 

Incentive: You can choose to be entered into a draw to win of two $50 VISA gift cards. Odds of winning depend on the number of people who have entered. To be eligible, you must have received Maltby Centre services in the last 12 months and provide a valid email address at the end of the survey.
