Elwha COVID Testing

The Lower Elwha Health Clinic is interested in learning more about the community's involvement with COVID testing. We have created a short anonymous online survey to help gather your input. This survey is 5-8 questions long (depending on what your answers are), and takes approximately 2 minutes to complete. The information we learn for this survey will help us better serve you with COVID-19 testing and antibody testing. Your participation is greatly appreciated and your feedback is highly valued and helps inform how we do things.

-Fever
-Cough
-Headache
-Muscle aches
-Runny nose
-Congestion
-Fatigue
-new loss of sense of taste and smell
-Sore throat
