Introduction to Local Health Assessment:

Sustainable Jersey Health Gold Certification is an achievement awarded to municipalities who show an exemplary effort in evaluating and addressing their own unique health and health equity needs. 

In pursuit of this certification, the Municipality of Princeton must conduct a community survey. Results of this survey will inform municipal plans to address the health needs of the community on issues including transportation, food, housing, wellness and recreation, safety and community belonging, public engagement and communication, and the COVID-19 pandemic. 

This is an anonymous survey and will not collect individually identifiable information. This survey has 18 total questions and it should take less than 10 minutes to complete.

Participation in this research is completely voluntary. If there are any questions that you feel uncomfortable answering, feel free to skip that question and leave the answer blank or stop the survey completely.

By completing this survey, you are granting your consent to participate in this research. 

All respondents may visit the Princeton Health Department office located at 1 Monument Drive, Princeton, NJ 08540 to receive a free swag item upon showing proof of completion of the survey.
