2026 Finger Lakes Region Nutrition Survey

This confidential survey helps us better understand nutrition and everyday challenges facing communities across the Finger Lakes. Past surveys have led to projects that make food access and healthy eating easier for your community.

You may enter a raffle for a $100 grocery gift card after taking the survey. A separate link will be given requesting preferred contact information to send the gift card and will not be connected with your survey responses.

If you have questions about this survey, you may contact jhatch@pivitalphp.org. Thank you!
