Welcome to the 'Community Access to Health Services' Survey

Aim
The aim of this survey is to gain an understanding of the factors that impact health service use in the Western Victoria Primary Health Network region so that we can better understand health needs and plan services for your area.

Who can participate?
Anyone over the age of 18 living in the Western Victoria Primary Health Network region. This includes the local government areas of: City of Ballarat, Central Goldfields Shire, Moorabool Shire, Hepburn Shire, Pyrenees Shire, Golden Plains Shire, Borough of Queenscliffe, City of Greater Geelong, Surf Coast Shire, Colac-Otway Shire, Glenelg Shire, Corangamite Shire, Moyne Shire, Southern Grampians Shire, Warrnambool City Council, Ararat Rural City, Hindmarsh Shire, Horsham Rural City, Northern Grampians Shire, West Wimmera Shire, Yarriambiack Shire.
 
Risks and benefits to you
Apart from the time it will take you to complete the survey, we do not foresee any risks to you. Your involvement is voluntary. We are not asking for any identifying information. There will be no direct benefits to you, but it is intended that the data that we collect will be used to inform planning, needs assessments and the commissioning of services.

How to participate
This project involves answering questions about health services in the Western Victoria Primary Health Network region. We would be grateful if you could complete this survey. It will take no longer than 5 minutes to complete. This survey will run from Tuesday the 11th September 2018 to Monday the 1st October 2018.

Consent
Completion and return of the survey will be understood to imply that you have given informed consent to participate in this survey and are over the age of 18.  If you decide not to participate there are no negative consequences. If you chose to participate, you may stop participating at any time and you may choose not to answer any specific question. Individual survey results will be kept confidential.
