Introductory Information about the Annual Survey

Why is the senior center doing this new survey?
It will collect data to clarify current needs and interests of older area residents and MSAC's role in responding to those needs/interests. MSAC is seeking national accreditation from the National Council on Aging, so the survey includes questions to help with that application. It should take about ten minutes to fill out.

Who can take part in this survey? It looks like one I saw a month or two ago.
Anyone age 50 or older and living in or near Washington County, Vermont. Recent one-time surveys by other groups explored specific issues, such as transportation. MSAC will conduct this survey every spring to help evaluate, plan and improve its services.

How do I take part in this survey?
You may fill out a paper survey and submit it to the MSAC office (58 Barre St., Montpelier) or complete the survey on-line. We prefer on-line submissions as they reduce our data entry time and effort. Link to the survey at www.montpelier-vt.org/msac/annualsurvey. Please get your completed on-line or paper survey to MSAC no later than Friday, June 7, 2019. Please call us at 802-223-2518 or stop by the office if you have questions.

What's in it for me?
In addition to helping MSAC improve its services, MSAC members who submit a completed survey by June 7 and attend the June 18 annual membership meeting may enter their names for raffle prize drawings taking place at the June 18 meeting! You must be present to win. Prizes will include TWO gift certificates worth $50 each toward MSAC services. The survey process will keep your answers confidential by having your name separate from your completed survey form. When you submit the survey on-line, a separate screen will pop up inviting you to identify yourself in order to enter the raffle. For people who fill out the paper survey instead, a separate raffle entry form is provided. Instructions for submitting the paper survey and raffle entry are on the back of the last page of this survey form.

How do I submit my completed survey? How do I enter the raffle?

If you are a new or renewing MSAC member, return/complete the survey by June 7 and identify yourself, your name will go into the hat for one of several prize drawings at the June 18, 2019, 12:45-1:45pm annual membership meeting at the MSAC facility. You must be present at that June 18 meeting to win. We hope to see you there and will share preliminary results of the survey at that time!

ON-LINE SURVEY: When you hit the "DONE" button below to "submit" your completed survey, please return to the MSAC website/survey page, and you will be able to link to and complete a separate short survey for the raffle prize drawing. If your having trouble, please contact the office for help to enter the raffle drawing and tell them you did your survey online.

PAPER SURVEY: If you fill out the paper survey and want to enter the June 18 raffle, please fill out the additional short form provided for that purpose and return it to MSAC with your completed survey. The survey form will be separated immediately from the raffle slip to protect your privacy. (If you prefer, you may return the form and the slip separately.)
