Nurse Aide & Nurse Long Term Care Study Informed Consent (part 1)
Description of the Study:
You are invited to participate in a new aspect of this research study. This research study is being conducted by Drs. William O’Brien, Steve Jex, Clare Barrett, and Russell Matthews, who are all Professors of Psychology at Bowling Green State University. Kristin Horan, Mary Moeller, and Sonia Singh, graduate students at Bowling Green State University, will also be helping out with the study. You must be at least 18 years old and currently employed as a nurse or nurse aide to participate in the study.
Summary of Involvement:
Thus far, you completed three surveys as a participant in this project:
The Baseline Survey: This survey will ask questions about your thoughts, feelings, and experiences relating to your work and home life.
Follow-up Survey # 1: This survey was similar to the baseline survey, asking questions about your thoughts, feelings, and experiences related to work and home life. You completed it about one month after the baseline survey.
Follow-up Survey # 2: This survey was identical the follow-up survey #1. You completed it about three months after the baseline survey.
In exchange for completing all surveys received $50 ($25 after follow-up survey # 1 and $25 after follow-up survey #2).
For this new aspect of the project, we are asking you to complete 3rd follow-up survey. This survey is identical to the ones you completed at baseline, follow-up #1, and follow-up #2.
Security:
If you choose to complete the electronic version of the follow-up survey, please be aware that (a) no form
of communication (e.g. email, telephone, or regular mail) is 100% secure, (b) employers or others may be able to track the information you type into a computer, (c) it is best to clear the browser cache and page history (see your browser instructions) after you complete the study.
Confidentiality: If you agree to participate, the researchers will carefully maintain your privacy. Only the researchers mentioned above will have access to your responses, which will be stored in a locked office at Bowling Green State University and on password-protected computers. Your responses will be assigned an alphanumeric code and identifying information will be deleted once your responses from each survey have been matched.
