Hospice Palliative Care Education

This is a standardized curriculum funded by the Ministry of Health and Long-Term Care (MOHLTC) through the Local Health Integration Network  (LHIN). It is recognized throughout Ontario.

The “FUNDAMENTALS” course is recommended for all Health Care Providers and anyone interested in becoming a Hospice Palliative Care Volunteer.

Topics will include:

* Introduction to Hospice Palliative Care
* Disease Management Domain
* Physical Domain
* Psychological Domain
* Spiritual Domain
* Social Domain
* Practical Domain
* End-of-Life Care and Death
* Management Domain
* Loss and Grief Domain
* Occupational Wellness
* Assessment Review (Enhanced)
* Pharmacological Review (Enhanced)

After 6 months experience in his or her role following active participation in the “Fundamentals of Hospice Palliative Care” education program, and with support from peers/managers, the learner will have the ability to influence the illness experience for the person and family to the extent that the learner:

1. Demonstrates sensitivity, understanding and respect for the individuality of the person, family and team/caregivers involved in the palliative experience.
2. Observes issues associated with illness and bereavement.
3. Effectively communicates with the person, family and team.
4. Recognizes and reports changes in any of the domains of issues associated with illness and bereavement (i.e. disease management, physical, psychological, loss/grief, social and practical, end of life/death management, and spiritual).
5. Provides palliative supportive care strategies.
6. Actively contributes to the team approach to hospice palliative care.

Course Expectations:

* 100% attendance and active participation in each session
* Satisfactory completion of assignments and module readings
* Respect for feelings and opinions of others is required
* Confidentiality is mandatory

Although there will be no formal exams, completion of program evaluation is required.

Additional Training:
The “Fundamentals of Hospice Palliative Care” has a 2-week “Enhanced” option, which is a prerequisite course for the Comprehensive Advanced Palliative Care Education (CAPCE) for NPs, RNs, and RPNs.

Note: The “Enhanced” option replaces Advanced Hospice Palliative Care Education (AHPCE) and is for NPs, RNs, and RPNs only.

Facilitators:
All classes are facilitated by a local or regional professional with teaching experience and expertise in the field of Hospice Palliative Care.
