Survey Disclaimer
The following questions will be used for general analytical use only. The purpose of the survey is to learn about the need, urgency, and impact (past, present, and future) of youth with disabilities seeking after-school and summer school care in rural areas. Although your email address is sent along with your answers, your specific responses will not be connected to you in any way whatsoever once posted on the website, social media, or any alternative form of media that Our Place After School Care Board sees fit.
After your results are added to the final tally, your email address will be deleted from any records we may have. We maintain the privacy and confidentiality of our survey participants with Respondent Anonymity Assurance (RAA). Once the RAA is activated, it cannot be removed or edited by the researchers. We appreciate your feedback as we work to better serve the community.
