Parent Substance Abuse Survey

The purpose of this questionnaire is to assess substance use in Escambia, Santa Rosa, and Okaloosa counties among residents ages 13-19. Your participation and the information you provide will help determine the need for substance abuse programs for youth. 

In this survey, the word “Substances” refers to illegal drugs, prescription medications (not prescribed to self), and alcohol.
1.Are you a parent or guardian of a child who is 13-19 years old?

4.During the past 12 months, how many times have you talked with your child about the potential dangers of drug use?
5.During the past 12 months, how many times, that you are aware of, has your child used substances?
6.During the past 12 months, have you removed substances from your child's possession? What substances were removed?
7.At what age did your child use a substance for the first time?
8.Do you feel that it is easy for adolescents to access substances at school or in the community?
9.Have you found online resources where you can learn more about how to help prevent your child from using substances?
