The Montana Patient-Centered Medical Home Program is conducting a survey of providers not in the program to learn more about your understanding, experiences and questions regarding PCMH. Your answers to the eleven questions below will help the CSI and stakeholder council develop strategies to assist non-participating clinics. Thank you in advance for your time and participation. (Only one survey per clinic is needed)
What is a PCMH? A patient-centered medical home is a team of healthcare professionals that transform their focus from treating illness to keeping patients healthy. A PCMH utilizes a “team” of primary care providers to coordinate all aspects of a patient’s health. The care team engages the patient as an active participant in their healthcare. PCMHs leverage technology to identify gaps in care for better disease prevention and management.
BackgroundIn April 2013, the Montana PCMH law was passed by the Montana Legislature. A PCMH healthcare provider must apply for qualification and receive approval from the Commissioner of Securities and Insurance before promoting itself as a medical home and receiving enhanced reimbursement from insurers and Medicaid. As of April 2016, there are 64 Montana PCMH clinics.
