As the American Academy of Pediatrics (AAP) continues to support its members and advocate for the health and well-being of children, Academy leadership and its new Task Force on Pediatric Practice Change are proactively exploring steps the organization can take to lead practice transformation for pediatrics and enable members to do the same.

The AAP wants to collect examples of innovative practice activities occurring among its membership. The information collected can provide a means to inspire and spread best practices for implementation by others.

In particular, the AAP is interested in hearing from members who have innovated in the following areas:
· Family engagement
· Utilization of technology, especially mHealth
· Telemedicine
· Implementation of team-based care
· Implementation of value-based or other non-standard payment models
· Population health approaches
· Integration/co-location with community-based services/organizations, including home visiting
· Consumer-driven approaches, including retail-based clinics, urgent care centers, on-demand care

Examples might include, but are not limited to, the following:
· Providing patients with enhanced access to after hours care using technology or formal collaboration with pediatric after hours services.
· Hiring parent partners or additional staff to provide care coordination or manage specific patient populations.
· Utilizing EHR data or other data to better understand the patient population and design specific interventions.
· Integration of behavioral health services or providers in the practice.
· Utilizing technology to provide remote visits to patients, or to remotely monitor patients with chronic conditions.
· Using social media to engage patients and families and provide education/peer support
· Providing home visits to targeted populations of patients.
· Implementation of new payment strategies.
· Utilizing peer navigators or community health workers to link patients/families with community and/or other resources
· Utilizing team-based care models.
