Consent

My name is Deeanna Burleson. I am a PhD student at Saybrook University located in Oakland, CA. I am conducting a dissertation research study to explore the lived experiences of integrative health professionals.  The research findings will be used to understand the critical dynamics and components needed for the IHC experience. Please read the following and if you agree to participate and consent indicate by entering your initials and clicking on the link to the online survey.  This survey will be used to select a diverse sample of 10 – 15 participants who are providing integrative healthcare.

Eligibility:
1. Working in an integrative health practice for at least the last 2 years.
2. Consent to participate.
3. Complete an online survey.
4. Agree to a one-on-one interview.
5. Agree for the one-on-one interview to be recorded.

 Activities:
If you participate in this research, you will be asked to:
1. Consent to participate.
2. Complete the online survey.
3. If chosen, schedule and complete a one-on-one interview.

Benefits:
As a part of the dynamic process of qualitative research you may experience an increased awareness of your part in promoting integrative healthcare.  You will also be providing critical information for the growth and strength of the integrative health model of care.

Risks:
There are no known risks to participating in this research.  

Confidentiality:
Your identity and the information you provide will be kept confidential. I will not identify you in any published research report or publication. I will keep all research information secure on a password-protected computer for seven years. After seven years the data will be appropriately destroyed. Only myself and possibly the dissertation committee will have access to your information.

Voluntary Participation:
Your participation is voluntary. 

Intervention:
There will be no experimental intervention.

Stopping Participation:
You are free to stop participation at any time without any penalty. If you decide to stop participation before this study is completed, send an email to dburleson@saybrook.edu requesting to withdraw from the research.

Audio-taping:
A voice recorder to record your responses will be used. Audiotaping allows me to concentrate on our conversation and not miss critical details of your answers or stories during the analysis phase of the research. Your recording will not be shared with others. The recordings are for my use to capture all that is said. You will not be able to participate if you do not wish to be recorded. 

Agreement and Consent:
Electronic consent is being used because of the following reasons:
1. Provide a smooth invitation to consent to online survey process.
2. Ease of participants.
3. Reduce the security risk in using and management of paper documentation.
