Community Engagement/SWOT Survey

You are invited to participate in our survey for the Florida Department of Health in Nassau County.  In this survey you will be asked questions to help us create a new Strategic Plan for 2020-2024.  We are requesting your feedback as it is very important to us. We value your partnership and services.  Help us identify gaps so we can provide quality public health services to Nassau residents. 

Our Services include: Community Outreach and Health Promotion, Dental Health, Disaster Planning, Disease Control, Environmental Health, Epidemiology, Healthy Start, HIV case management, Personal Health (medical, immunizations, family planning), Medical Referral Programs, School Health, Volunteer program and WIC (special supplemental nutrition for Women, Infants, and Children). 

The ten essential public health functions include: monitoring health, investigating disease/outbreaks, educating, mobilizing the community, policy & health planning, enforcing health laws, linking to health services, assuring health workforce, program evaluation, public health research and innovation.
