Organization Members Experience Survey

Thank you for providing feedback on the services and leadership of StageSource. Please note that this survey is intended for organizational members of StageSource; there are separate surveys for non-members (link) and individual members (link). When you complete this survey, you will be provided the password and link to enter a drawing for a $50 Visa gift card. Entry to the giveaway is not associated with your survey responses.

Survey responses are strictly confidential and will not be shared with StageSource Staff or Board. H.M. Carter Consulting will use combined data from all responses to identify patterns in responses as foundation for reports and recommendations, and any potentially identifying information will be removed.

This survey is completely voluntary. You do not have to participate in this survey and – if you do choose to participate – you may skip any question you do not feel comfortable answering or which does not seem applicable.   You may exit the survey at any time.    The survey should take about 10 – 15 minutes to complete.  We would ask that you complete the survey by Wednesday June 15, 2022. 

Some survey questions refer to programs which have not occurred recently, or have occurred in an online format for public safety. Please answer as best as you are able based on the length of your experience with StageSource.
