1. About the survey

Every two years, FCO conducts a survey to find out more about the Family Councils, Council members, and long-term care home staff in Ontario. We strive to offer the kinds of programs and services that will benefit you and your home's Family Council. Our Family Council 360 Survey is one way we listen to your needs and experiences to inform our work and ensure that our programs and services meet your needs.

Your input matters to us! As a result of the 2017 survey, FCO introduced a conflict resolution training program, launched a committee to help us evaluate and update our education and training materials, and used the suggested learning topics to plan our events.

New this year, help us learn more about Ontario's Family Councils and what you need from us to enhance your effectiveness. For the first time, Family Council 360 features a separate staff survey. We want to know about your relationship with your home's Family Council and what you need in order to form effective and collaborative relationships with Council members. We'll use this info to plan our work and inform our conversations with government and community stakeholders.

Tell us what you think of the content, appropriateness, format, and delivery of our education and training materials so we can ensure that they meet your needs and those of your Council. Provide us with feedback on what we're doing well and what we can do better.
All responses are voluntary. Only aggregate, summary data will be made public.

Thank you in advance for your feedback!
