Westlock Minor Hockey Coach Evaluations - Parent Questions

The purpose of this Survey is to assist our association for the development  of our coaches. Please be honest in you assessment in both constructive criticism and praise.  You are encouraged to talk with your child as you respond to the questions.
Your input is essential to improving the quality of the coaching, and in our coach selection process.  Thanks for your input into your association.
