You are invited to participate in a web-based online survey on inclusion in your community. We want to hear about your inclusion in your community and how we can help businesses better meet your needs. This is a market research project being conducted by FAVE Solutions. It should take approximately 10 minutes to complete.

PARTICIPATION: Your participation in this survey is voluntary. You may refuse to take part in the research or exit the survey at any time without adversely affecting your relationship with anyone associated with FAVE or FACES. You are free to decline to answer any particular question you do not wish to answer for any reason.

BENEFITS: You will receive no direct benefits from participating in this research study. However, your responses may help FAVE facilitate inclusion for individuals with communication and intellectual disabilities within businesses and organizations in your community.

RISKS: The possible risks or discomforts of the study are minimal. You may feel a little emotional when answering survey questions about community inclusion.

CONFIDENTIALITY: Identifiable information will be kept strictly confidential, and digital data will be stored in secure computer files. Any report of this survey that is made available to the public will not include your name or any other individual information by which you could be identified.
If you choose to provide contact information such as your email address, your survey responses may no longer be anonymous. However, no names or identifying information would be included in any publications or presentations based on these data, and your responses to this survey will remain confidential. You may print the consent form and survey for your records.
